Addiction and Eating Disorder Treatment


Eating disorders are obviously food-related illnesses. It is also important to note that often those with anorexia or bulimia have substance-abuse issues. The drug of choice may be legal or illegal, but the end result is still the same: abuse and/or addiction.

Stimulants are often abused by those with eating disorders, especially anorexia. Women or girls rely on large amounts of caffeine, such as that found in black coffee or diet drinks, to stave off hunger. Often, they take up smoking, looking to nicotine to serve as an appetite suppressant. Those with bulimia may turn to amphetamines and cocaine to provide energy and also decrease hunger. Remuda Ranch Offers its Support of Eating Disorders Coalition Lobby Day on Capitol Hill

PHOENIX, Sept. 17 /PRNewswire/ -- Since 1990, Remuda Ranch Programs for Eating and Anxiety Disorders has treated more than 10,000 patients suffering from anorexia, bulimia, anxiety and related issues. On Saturday, September 30, Remuda Ranch will support the Eating Disorders Coalition's (EDC) efforts on Capitol Hill. Eating disorders are unique to each person. We have treatment options that are unique as well.
The scope of care for eating disorder patients depends on the individual and their severity level. At Eating Disorder Center of Denver, we have five levels of treatment that serve most eating disorder patients:

* Partial Hospitalization Program (PHP) – Our highest level of care for patients diagnosed with an acute eating disorder. This highly structured program includes therapy and therapeutic activities 10 hours a day, seven days a week.
* Intensive Outpatient Program (IOP) – A step down from PHP. Patients transition to some eating on their own and some time away from the center. This program is approximately four hours each day.
* BRIDGE Evening Intensive Outpatient Program (BRIDGE EIOP) – For patients who require more than outpatient therapy to treat their eating disorder. This program is considered a “transitional” level and includes therapy four evenings per week (includes meals) for 10 weeks.
* Outpatient Service/Aftercare and Support Groups – Offers a more personal connection than a support group can provide. Therapists, registered dietitians and psychiatrists provide the necessary support to maintain health and recovery.

We are pleased to announce the 2011 Grant Cycle for the Klarman Family Foundation Grants Program in Eating Disorders Research. The Program's short-term goal is to support the most outstanding science and expand the pool of scientists whose research explores the basic biology of anorexia nervosa, bulimia nervosa, and/or binge eating disorder. The long-term goal is to improve the lives of patients suffering from these conditions.

Examples of funding areas include but are not limited to molecular genetic analysis of relevant neural circuit assembly and function; epigenetic research; animal models created by genetically altering neural circuits; and testing of new chemical entities that might be used in animal models as exploratory treatments. Please note that imaging studies will NOT be accepted in this grant cycle. Investigators conducting research in the neuro-circuitry of fear conditioning or reward behavior may also apply but must justify the relevance of their research projects to the basic biology of eating disorders. Clinical psychotherapeutic studies, medication trials and research in the medical complications of these disorders are outside the scope of this Program.

The Klarman Family Foundation Grants Program in Eating Disorders Research is administered by The Medical Foundation, a division of Health Resources in Action, a nonprofit Boston-based organization.

Re-Enchantment Day ~ Prominent innovation at the Eating Disorders Program, Texas Health Presbyterian Hospital Dallas

In our stress filled lives individuals find themselves endlessly distracted from the beauty, wonder, and spiritual nature of life. We at the Eating Disorders Program at Texas Health Presbyterian Hospital Dallas see this issue particularly affecting those struggling to overcome an eating disorder. So our treatment team developed a special program to complement our evidence based treatment approach and specifically address the need to reconnect to the spiritual nature of life: Re-Enchantment Day.

Re-Enchantment Day is a full day of intensive programming designed to raise awareness of our spiritual selves. Our multidisciplinary program staff actively participates in this process right alongside those in our program struggling to overcome their eating disorders, thus eliminating artificial barriers and opening connections at a universal level. The following description provides some of the highlights of the experience, though does not do justice to the profundity of the event.

Re-Enchantment Day begins with an opening prayer and breakfast dining in elegant fashion on exquisitely designed purple table cloths with fine china and elaborate figurines embodying the cycle of life: youth (spring), adulthood (summer), middle age (autumn), and older age (winter). Names of powerful women of both historical and current stature, along with a brief description of their contributions, are individually written on sheets of paper and taped to people’s backs. Lively discussion ensues as people use 20 questions to identify whose name is written on their back. This serves to connect us with a sense of greater purpose, transcending what some refer to as the mundane of everyday life.

Our chaplain leads a silent noon meal which initiates with a sacred and inspiring reading and introduces the idea that silence provides an opportunity to listen to an inner voice without distraction. At first most people are skeptical of this exercise yet are shocked to discover what develops out of the experience.

A unique art therapy task has the group build a life-size sculpture of a woman. Amazingly similar details emerge in the finished sculpture between different groups going through this exercise at different times having had no contact between the groups. One may begin to comprehend the universal and powerful nature of woman’s spirit.

The day ends with a Trust Walk. This experiential exercise requires a level of trust in another to protect one while walking blind folded. After placing the blind fold and taking an arm for initial guidance, one may let go of the arm and rely on verbal directions to avoid pitfalls. We don’t walk alone: our life paths appear to develop more fully when based on trust and with awareness of serving a purpose greater than ourselves.

If you treat patients with eating disorders, you know they struggle with issues that are both complex and pervasive. This unique 2-part workshop addresses the medical, neurophysiological and psychological issues prevalent in this population. The workshop offers practical skills and tools to treat patients with eating disorders.

Part 1: Dr. Kevin Wandler (10:00 am - 12:00 pm)

Our weight-obsessed culture places a person’s value and worth on their external shape and size, creating a hostile recovery environment for those suffering. This portion of the workshop will address the medical complications of eating disorders and pharmacologic therapies for treating these disorders and co-occurring disorders. Further, acceptance and change strategies blended with life-management skills offer a useful and practical intervention for meeting the needs of eating disorder patients. Through case studies and experiential exercises, this presentation includes tools, assignments, and strategies that have been effectively utilized in the treatment of more than 10,000 Remuda Ranch patients.

Following this presentation, participants will:

  • List 3 medical and psychiatric complications of eating disorders.
  • List common co-occurring diagnoses seen in patients with eating disorders.
  • Describe issues in treating males with eating disorders.
  • Apply acceptance and change strategies to the most difficult emotional and behavioral issues.
  • List 3 techniques of DBT skills you can use to help with emotional dysregulation.
  • Describe the core body image disturbances, cognitive errors, distortions, and identity issues interfering with recovery.

Part II: - Dr. Ralph Carson (12:45 pm - 3:00 pm)

How do dysfunctional behaviors (starvation, binging, purging, cravings) trigger compulsions and ultimately hijack the brain? The roots of addiction and eating disorder behaviors lie in a complex chain reaction that originates in an ancient survival mechanism of the brain where this system is inappropriately activated and drives the body to crave, sometimes with addictive behavior as the end result. This presentation will not only investigate how chemicals directly interact with brain chemistry to produce behaviors, but how cognitions alone can produce similar responses. Additionally, the talk will expound on how exercise and nutrition can directly act on our unique neurochemistry to change the landscape and correct imbalances.

At the end of the sessions, participants will be able to:

  • Outline the neuropathology of eating disorder behavior.
  • List the ways such compulsions are similar genetically and neurochemically.
  • Identify pathways through which feelings are transformed into neurochemical messages.
  • Suggest substitutes for food cravings that elicit equal neurochemical responses.
  • List three ways that exercise enhances brain function.

Speakers:

Ralph Carson, PhD, RD has been involved in the clinical treatment of obesity, addictions and eating disorders for more than 30 years. His unique background in health science and medicine (BS Duke University and B.H.S. Duke University Medical School) coupled with nutrition and exercise (BS Oakwood College, Ph.D. Auburn University) has prepared him to integrate biophysiological intervention and proven psychotherapeutic treatment. Dr. Carson is an acting board member of the International Association of Eating Disorder Professionals and a partner with Remuda Ranch Programs for Eating and Anxiety Disorders. In this capacity, he educates Remuda Ranch staff and those working in the field of eating disorders about the latest in treatment innovations and research. Dr. Carson is the author of the book Harnessing the Healing Power of Fruit: The New Paradigm for Optimum Health. His new book, The Brain Fix will be released later this year.

Kevin Wandler, MD, CEDS is board certified in General Psychiatry, with Added Qualification in Addiction Psychiatry, by the American Board of Psychiatry and Neurology. Dr Wandler has been the Chief Medical Officer at Remuda Ranch Programs for Eating and Anxiety Disorders for over 14 years. He has presented frequently to US and international audiences on state-of-the-art treatments for eating disorders and co-occurring disorders. Today, the Body Image Institute of New York (BIINY) announced its support for National Childhood Obesity Awareness Month. Throughout the month of September, The Body Image Institute will hold seminars and workshops to raise awareness about the obesity epidemic in an effort to help improve the health of American children.

Childhood obesity has been a growing problem over the past four decades. Currently one out of every three (31.8%) children living on American soil, ages 2 to 19, is overweight or obese; a statistic that health and medical experts consider an epidemic.

Being overweight or obese is unhealthy at any age. Obese children are at risk for a number of emotional, physical, life-threatening, and psychological problems. They are often teased by their peers, and harassed or discriminated against by their own family and friends. These children may grow into individuals with low self esteem, which predisposes them to depression.

The life-threatening consequences of this epidemic create a compelling and critical call for action. Obesity is estimated to cause 112,000 deaths per year in the United States, and one third of all children born in the year 2000 are expected to develop diabetes during their lifetime. Experts predict that the current generation may be on track to have a shorter lifespan than their parents.

Childhood obesity as a condition also places those affected at greater risk of developing heart disease and cancer. It can also lead to other conditions such as osteo-arthritis, sleep disorders, high cholesterol and other disorders that may include liver disease, early puberty or menarche, eating disorders such as anorexia and bulimia, skin infections, asthma and other respiratory problems. Studies have shown that overweight children are more likely to become overweight adults. Obesity during adolescence has been found to increase mortality rates during adulthood.

While obesity afflicts children across the US, certain groups have been disproportionately affected. For instance the Bronx is ground zero for childhood obesity in America. Almost half (47%) of Head Start participants in the South Bronx are either obese (31%) or overweight (16%). Nearly 4 in 10 (39%) kindergartners and 5th graders attending public schools there are either obese (24%) or overweight (15%). A similar proportion (38%) of high school students reports heights and weights that indicate obesity or overweight; 17% are obese, and 21% are overweight. Among adult residents, 2 in 3 (66%) report heights and weights indicating they are obese or overweight.

It is undisputed that the Bronx, as a city, has the largest number of obese infants in the US. Obesity rates are also highest among non-Hispanic black girls and Hispanic boys. Obesity is particularly common among American Indian/Native Alaskan children. In addition, obesity can be influenced by a number of environmental and behavioral factors, including unhealthy eating habits and a sedentary lifestyle at home and at school.

According to Marie-Ange D. Tardieu, M.D., medical director of the Body Image Institute of New York and author of The Elements of Size: Issues in Body Image, Obesity, Overweight, Eating and the Mediterranean Diet, prevention of obesity must begin early and continue throughout life with particular attention directed to three critical periods in a child’s development: before birth, between the ages of 4 and 7, and during puberty.”

Dr. Nereida Correa, who has served as a member of the National Advisory Council for Maternal, Infant and Fetal Nutrition, is on the Advisory Council of Women's Health of the Health and Human Services (HHS) Administration in Washington DC, and is the president and CEO of Eastchester Medical Associates concurs: “advice about diet and exercise should be dispensed throughout childhood” … “obesity prevention should start when the child is in its mother’s womb … before delivery, breast feeding should be encouraged because it helps prevent obesity in infancy and offers a protective factor against obesity in later childhood. (A 60 percent increase in obesity has been observed in children who were not breastfed.)”

Stephanie C. Tardieu, who reviewed the effects of consuming sweets in early life noted: “ 70 percent American infants receive sweets by the age of 15 months, and by the age of 2, 43 percent of American toddlers are given soft drinks daily. (“One can of a non-diet soft drink contain 150 calories, the equivalent of 10 spoons of sugar.)” Miss Tardieu further remarked: “As responsible adults, we can all join forces and take steps to turn the epidemic of childhood obesity around, and guide our communities to healthy eating and lifestyle habits.”
